TWO letters of reference must be submitted in support of the Pharmacy application, using the ‘Structured Reference Letter’ form (download this form via the link below). The two reference letters must be submitted by the deadline of JANUARY 31, 2013.

Please mail the completed form to the UBC Faculty of Pharmaceutical Sciences, Undergraduate Programs, 2405 Wesbrook Mall, Vancouver, BC, V6T 1Z3. If this form is not received in our office or post-marked by January 31st, your application will be considered incomplete.

PLEASE NOTE: We do not begin to match the supporting documents, i.e. reference forms and PCAT scores, to the application, until after the application deadline.  Your status may show “not received” until then.  It will take us a couple of months to work through the documents to match and update everyone’s file.  Also, as stated on our online application webpage at: https://secure.dentistry.ubc.ca/pharmacy/application/instructions.asp: “ After you have submitted your application you will be able to view a checklist of your supporting documents that have been received.  The checklist will be updated by March 15th.” . 

If for any reason we do not have your reference form(s) and PCAT score we will notify you immediately.
